What is a Patient Participation Group?

The Patient Participation Group (PPG) is a group of patient volunteers who attend meetings to promote health and to help support and improve the planning, provision and delivery of local NHS services. The Group aims to support the highest quality of care for all patients by representing the views of the patients. This is achieved by working in partnership with the Surgery and their staff. The group is a positive and supportive forum to represent the views of the members of the group and also to represent the wider patient population.

What is a Primary Care Network?

Primary Care Networks (PCN) are a key part of the NHS Long Term Plan. They consist of groups of general practices working together with local providers such as community services, social care and the voluntary sector, to respond to the needs of the patients they serve.

The network aims to focus on population health and addressing health inequalities in their local area, providing a more personalised, coordinated and social care services to their local population.

Social Prescriber

Social Prescribing link workers help to reduce health inequalities by supporting patients to unpick the complex issues that are affecting their wellbeing.
They enable people to have more control over their lives, help patients develop skills and give their times to others, through involvement in numerous community groups.

Clinical Pharmacist

Clinical Pharmacists are responsible for the care management of patients with chronic diseases and those on multiple medicines. They proactively undertake clinical medication reviews to improve the quality of care received and to ensure patients safety.
