Our patient’s safety during this period remains our top priority which is why appointment only access to our buildings remains in place. However, all of our practices remain open. If you have any concerns, please contact your GP.
CCPCN COVID-19 Vaccination Programme
This is a great achievement and would not have been possible without the hard work and dedication that we have seen from our staff and also from the many volunteers that have helped out at the vaccination centre.
With the support of PCN staff, volunteers and working collaboratively with Home Instead, Fountains Care Home Service successfully vaccinated the majority of care home patients and staff before the 14th February deadline.
New Roles for 2021
We are currently exploring the following roles to expand our PCN multi-disciplinary team (MDT) and support our PCN in delivering high quality patient services.
Joining our team from April 2021, this role will assist with a range of tasks within our practices from preparing repeat prescriptions to undertaking prescribing audits and helping patients to get the best outcomes from taking their medicines.
We have a Care Co-ordinator joining our team in April 2021. Working closely with our MDT and with the GPs, the care co-ordinator will work with identified patients to support their personal care requirements and ensure that appropriate support is made available.
Mental Health Practitioner
We are currently working with our community mental health provider in an effort to increase the support we can give to our patients with mental health needs. We hope to have a mental health practitioner in place in April which will be additional to the low-level mental health service we already provide.
We are currently recruiting for the role of Paramedic within our PCN. The role of the paramedic will be to provide holistic care for individuals which will include assessment, diagnosis, prescribing and treatment. This includes home visiting, clinic working and also patient caseload management.
We are exploring the role of Nursing Associate within the PCN. Nursing associates work with people of all ages, in a variety of settings and contribute to the core working of nursing.
